The Level of Care Assessment (LOCA) is the assessment tool used to determine and document each consumer’s level of care.  LOCA is the initial assessments tool for consumers seeking Medical Assistance payment for nursing home care, the SSI supplement for Personal Care and Domiciliary Care and all home and community based care managed services (PDA Waiver, OPTIONS and Family Caregiver Support Program).  In addition to determining the appropriate level of care for each consumer, the findings of this assessment are used, in part, to develop the consumer’s plan of care.
